Prisoner Ombudsman says death of Maghaberry overdose inmate could have been prevented

The Prisoner Ombudsman says the death of a man from a drug overdose last year could have been prevented.

Paddy Kelly, 46, died in March last year after hoarding prescription drugs while on remand in Maghaberry Prison. He was known to be at risk of self-harm.

Prisoner Ombudsman Tom McGonigle's report said Mr Kelly's overdose was "foreseeable".

The health trust in charge of his care said actions have been taken to address the "practice issues" identified.
